Perhaps you have already come into contact with this phenomenon yourself? Because around half of people with Parkinson&#8217;s are familiar with the sudden freezing of gait, which can usually occur for the first time from the fifth year after diagnosis. In moments of freezing, nothing happens for several seconds. A feeling of paralysis and loss of control over movements is described because the feet can only be lifted from the ground with great difficulty or not at all.    <\/p>\n<p>Freezing can last from a few seconds to half a minute. Sometimes the movement block occurs sporadically, sometimes several times a day. The blockage occurs while the patient is fully conscious and is a stressful experience for many of those affected, as their own body suddenly refuses to move in the usual way. They describe the condition of those affected in relation to the effect of the medication: <\/p>\n<ul>\n<li><b>On phase: <\/b>In the on phase, the medication, usually levodopa, works well. The symptoms are significantly alleviated and movements are easier. During this phase, many patients are able to cope with everyday life with virtually no symptoms. Freezing also occurs less frequently in the on phase or is less pronounced.   <\/li>\n<li><b>Off-phase:<\/b> In the off-phase, the effect of the medication wears off and the Parkinson&#8217;s symptoms become more severe. In this phase, movements become more difficult, tremors and stiffness increase, and freezing occurs more frequently and more intensely. <\/li>\n<\/ul>\n<\/div><\/div><\/div><\/div><div class=\"fusion-text fusion-text-5\" style=\"--awb-font-size:18px;--awb-text-color:#221c1e;--awb-text-font-family:&quot;Libre Franklin&quot;;--awb-text-font-style:normal;--awb-text-font-weight:400;\"><h2>What are the causes and triggers of freezing?<\/h2>\n<p>Freezing &#8211; this sudden freezing of movement &#8211; is not the result of weakness or a lack of willpower, but the result of complex changes in the brain that are typical of Parkinson&#8217;s disease. Knowing that the brain can no longer make small adjustments as usual helps us to understand these moments better and to show ourselves a little more compassion. Possible causes and triggers of freezing include:  <\/p>\n<ul>\n<li><b>Dopamine deficiency<\/b>: In Parkinson&#8217;s disease, the brain is no longer sufficiently supplied with the neurotransmitter dopamine. However, dopamine is important for smooth movements. If the dopamine level drops, movements may suddenly be interrupted. This is often the case during an &#8220;off-freezing&#8221; phase, when the effect of the medication wears off.   <\/li>\n<li><b>Effect of the medication<\/b>: Sometimes freezing is also caused by the effect of the Parkinson&#8217;s medication itself. Paradoxically, if the medication has a particularly strong effect, this can also lead to freezing. This is known as &#8220;on-freezing&#8221; (&#8220;on phase&#8221;) and can be particularly frustrating because the treatment that is supposed to help can have the opposite effect in some situations.  <\/li>\n<li><b>Stressful situations<\/b>: Situations that require extra concentration, such as crossing a road or entering a confined space, can also trigger freezing. Stress, uncertainty and the feeling of &#8220;having to perform particularly well now&#8221; often increase the risk of freezing. <\/li>\n<li><b>Environment and sensory impressions<\/b>: Frequent changes in the environment, for example lots of people, noises or sudden obstacles, can promote freezing. Unfamiliar surfaces or narrow passages can also have an effect, as the brain is sometimes overwhelmed by the many impressions. <\/li>\n<li><b>Patterns in the brain and habitual movements<\/b>: Movement sequences are deeply anchored in the brain. If these processes are disrupted by Parkinson&#8217;s, it can happen that the habitual patterns seem &#8220;blocked&#8221;. The brain then no longer sends a clear signal to the legs, causing them to virtually stand still. This shows that freezing is often not simply a &#8220;head thing&#8221;, but is based on complex processes in the brain.   <\/li>\n<\/ul>\n<p><b>Good to know:<\/b> A combination of several of these causes is often a possible trigger. Whatever triggers freezing, it is helpful to know that with practice, patience and professional support, you can find ways to overcome such moments. <\/p>\n<h2>Symptom &#8220;freezing&#8221; in Parkinson&#8217;s disease: how the blockage manifests itself<\/h2>\n<p>Freezing can manifest itself in various moments and situations. Here are typical signs of movement blockage: <\/p>\n<ul>\n<li><b>Start inhibition<\/b>: Difficulty starting to move, especially after a break in walking, when the first step is to be taken.<\/li>\n<li><b>Sudden blockage of movement when walking<\/b>: When crossing narrow areas, such as door frames or corridors, when entering elevators or when turning or changing direction, the movement sequences freeze.<\/li>\n<li><b>Stress and anxiety situations<\/b>: Freezing is more common in moments of increased tension or in unfamiliar environments.<\/li>\n<li><b>Influence of medication<\/b>: Freezing can occur in connection with certain phases of medication intake, which can possibly increase the blockages.<\/li>\n<\/ul>\n<h2>Complications of freezing in Parkinson&#8217;s disease<\/h2>\n<p>Freezing is not only an unpleasant symptom for people with Parkinson&#8217;s, but also poses various risks and dangers in everyday life. The sudden blockage of movement leads to those affected feeling &#8220;at the mercy&#8221; in certain situations, which can lead to insecurity and frustration. These moments of freezing are particularly problematic in everyday life if they occur at critical moments, for example when crossing roads or getting on and off public transport. Below we summarize the most important risks and complications that can arise from freezing:   <\/p>\n<ul>\n<li><b>Increased risk of falling<\/b>: Suddenly stopping a movement can severely disrupt balance. Especially when walking or turning, the sudden stop may cause those affected to lose their balance and fall. The risk also increases if freezing occurs when walking downhill, such as on stairs or sloping paths &#8211; a sudden stop can cause particularly serious balance problems.  <\/li>\n<li><b>Risk of injury from falls<\/b>: There is an increased risk of injury from falls caused by the sudden blockage of movement. Abrupt movements and falling can lead to bruises, broken bones or even head injuries. Such injuries can lead to a considerable impairment of mobility in older people.  <\/li>\n<li><b>Loss of independence and mobility<\/b>: Frequent freezing not only has a physical impact on life, but also an emotional and practical one. The sudden freezing moments in everyday life, such as when walking around the home or on the way to the shops, can make the person affected feel insecure and increasingly dependent on relatives or carers. This restriction of mobility then also significantly limits their independence.  <\/li>\n<li><b>Psychological stress<\/b>: The unpredictable occurrence of freezing episodes can trigger severe anxiety, e.g. fear of movement (kinesiophobia). This fear often leads to increasing avoidance of activities, which in turn has a negative impact on self-confidence and general well-being. The fear of suddenly &#8220;freezing&#8221; in a risky situation, such as when crossing a road, can lead to those affected increasingly restricting their range of action.  <\/li>\n<li><b>Restrictions on quality of life<\/b>: The constant uncertainty caused by freezing and the associated complications can significantly reduce quality of life. As those affected may avoid activities and meetings with friends and family in order to avoid unpleasant situations, the risk of social isolation increases. The result can be a withdrawal from social life, which in turn significantly impairs life satisfaction.  <\/li>\n<\/ul>\n<h2>Diagnosis of &#8220;freezing&#8221;<\/h2>\n<p>The diagnosis of freezing in Parkinson&#8217;s disease is based on a thorough clinical examination and close observation of the symptoms by experienced physicians. The focus is placed on the specific characteristics that are typical of freezing: <\/p>\n<ol>\n<li><b>Medical history and questioning<\/b>: First, the doctor will conduct a detailed interview to establish the frequency, timing and circumstances of the freezing. They will also ask about triggers such as confined spaces, sudden turns or situations such as starting or stopping walking. <\/li>\n<li><b>Observation in everyday life<\/b>: Certain movements that can trigger freezing are often tested, such as walking, turning or walking through door frames. The doctors observe whether the typical &#8220;freezing&#8221; occurs and how long it lasts. <\/li>\n<li><b>Special tests and questionnaires<\/b>: There are standardized tests and questionnaires, such as the FOG score, which help to assess the extent and frequency of freezing. These tests can also provide valuable information about the severity and specific triggers of the symptom. <\/li>\n<li><b>Movement analysis using modern technology<\/b>: In some cases, modern technology such as wearable sensors or video analysis is also used to precisely record the gait pattern. This allows doctors to recognize subtle signs of freezing that sometimes go unnoticed in everyday life. The progression and effectiveness of treatment measures can also be documented.  <\/li>\n<li><b>Motor tests<\/b>: Standardized motor tests such as the Unified Parkinson&#8217;s Disease Rating Scale (UPDRS) help to objectively assess the ability to move and the extent of freezing.
